MDOUTLOOK, the global oncology intelligence initiative powered by The Arcas Group, announced results from its 4th Annual Quick Poll research launched immediately following this month’s annual meeting of the American Society of Clinical Oncology (ASCO) in Chicago. These Quick Polls expand on previous Quick Polls carried out after the past 3 ASCO annual meetings, and other meetings such American Society of Hematology (ASH) and San Antonio Breast Cancer Symposium (SABCS).
Within one week, 515 cancer treaters in 38 countries responded to 4 individual Quick Polls on melanoma, non-small cell lung cancer (NSCLC), gastrointestinal (GI) cancers and genitourinary (GU) cancers. This is the first market feedback available post-ASCO. Highlights from this significant research study include:
- News from ASCO impacts the entire oncology community. A wide variety of sources are used to learn about the important news from this conference.
- The novel targeted agents vemurafenib and ipilimumab will become the standards of care for malignant melanoma. More than half of all patients with malignant melanoma are expected to receive one if not both of these agents.
- Physicians are excited about the identification of specific NSCLC patient subpopulations who will respond to crizotinib, erlotinib, or MetMab.
- Following their recent regulatory approvals, worldwide use of everolimus and sunitinib for pancreatic neuroendocrine tumors (PNET) will greatly increase.
- There is not a consistent approach to the continued use of anti-angiogenic therapy after progression in stage IV colorectal cancer. Physicians are eagerly awaiting the results of definitive clinical trials which are due in the near future.
- Axitinib is expected to be the most common 2nd line therapy following sunitinib for metastatic renal cell carcinoma (mRCC). However, clinicians use a personalized approach in this setting.
- The clinical landscape for prostate cancer is undergoing a dramatic shift due to a number of new therapeutics; some of which have been recently approved and others still under clinical development.
- The primary clinical trial endpoint reported influences the integration of research into clinical practice.

A number of current events are going to have a significant impact on the treatment of metastatic breast cancer in Europe: the EMA has recently reduced the marketing approval for bevacizumab and new positive data from the EMBRACE trial (eribulin in the recurrent setting) were made available.
MDoutlook launched a Quick Poll™ among its 12,000+ European medical oncology members to determine how these events are and will be affecting clinical practice. Responses from 13 countries (>60% of responses were from the 5-EU) show the following:
• Initial uptake in usage of eribulin will be stronger in later lines of treatment, although a majority of oncologists will begin using eribulin in the 2nd line for some of their metastatic breast cancer patients (see figure below)
• It is widely believed that eribulin will soon prove to be effective as an initial treatment for metastatic breast cancer with less acceptance as a neoadjuvant or adjuvant agent
• Significant reductions in the use of bevacizumab + docetaxel in metastatic breast cancer have recently been made by oncologists; further reductions in the use of this combination will occur due to the EMA’s decision
• The use of bevacizumab + paclitaxel, which was unaffected by the recent EMA decision, has had and will have only some minor reductions in usage for metastatic breast cancer. The use of bevacizumab in other tumor types are also relatively unaffected

Last Thursday, two articles in the New England Journal of Medicine described exciting results with sunitinib (Sutent®) and everolimus (Afinitor®) from phase III clinical trials in pancreatic neuroendocrine tumors (PNET). To gauge the immediate impact these reports could have with the global oncology community, MDoutlook launched a global Quick Poll™ of its medical oncology members. Based on the initial results collected over the weekend:
• While 40-50% of oncologists are already using sunitinib or everolimus for the treatment of some of their patients with PNET, over 80% plan on using them in 2011
• The amount of patients with PNET who will receive sunitinib or everolimus for treatment in 2011 is predicted to expand by 2.5-3.0 fold over current levels (see figure below)
• Three quarters of oncologists expect their PNET patients will stay on extended therapy with these agents regardless of the potential for serious side effects
• Usage of these agents in PNET will have little to no effect on their use in metastatic renal cell carcinoma (where both currently have FDA-approval).

MDOUTLOOK® Quick Polls from the Major Oncology/Hematology Meetings ASH and SABCS: First Report from Cancer Treaters on Clinical Impact of New Cancer Developments
Atlanta, GA, United States – December 21, 2010
MDOUTLOOK®, the oncology intelligence initiative powered by The Arcas Group, today announced results from its series of immediate Quick Poll research launched following major oncology and hematology market moving events. These Quick Polls examined the potential clinical impacts from the American Society of Hematology (ASH) annual meeting in Orlando and the 33rd Annual San Antonio Breast Cancer Symposium (SABCS). This effort expands on previous Quick Poll research carried out after the American Society of Clinical Oncology (ASCO) annual meetings in 2009 and 2010.
Within one week, 506 cancer treaters from over 30 countries responded to the Quick Poll research following the ASH and SABCS meetings, making this the first market feedback available post-ASH and post-SABCS. Some of the highlights from this research include:
• Decisions concerning the use of drugs are primarily made independently of any indications / approvals in different malignancies, even if it is in a related tumor type.
• Maintenance therapy with lenalidomide is seen as a highly important advance in the management of multiple myeloma. It is expected that half of all patients with multiple myeloma will receive this therapeutic approach in 2011.
• There appears to be a new standard of care for mantle cell lymphomas: alternating cycles of CHOP and DHAP (both with rituximab), followed by high dose ARA-C and autologous stem cell transplantation.
• Oncologists / Hematologists are not differentiating primary from secondary AML in their outlook for new therapeutics.
• New data showing a lack of association between CYP2D6 phenotype and tamoxifen responsiveness will cut in half the amount of CYP2D6 genotyping being performed in postmenopausal women with HR+ breast cancer from 11 to 5%.
• Dual targeting the HER2 receptor is a very important clinical advance for HER2+ breast cancer. The use of this strategy in appropriate breast cancer subtypes with currently available therapeutics is expected to double in 2011.
Members of the MDOUTLOOK Strategy Council reflected that these results “show the quick and strong uptake of new developments presented into physician decision-making on a global scale” and that “it will be of value to follow-up on a regular basis to track these changes.”
